We participate with Medicare, Blue Cross/Blue Shield, Aetna, United Healthcare and other major health insurance companies. Please bring your card so that we may obtain a copy. If you do not bring your card you are expected to pay in full or be rescheduled. Co-payments are collected prior to your visit.
If you have insurance with an HMO, PPO or Managed Choice, check your handbook and see if we are a participating provider. If our physician does participate:
- Bring your insurance card.
- If you fail to bring your card, you will be required to pay in full.
- Bring your co-payment for each visit, payments are collected when you sign in.
- For HMO’s, obtain the proper referral number/form from your primary care physician for each visit.
If our physician does not participate with your insurance company, we still need to get a copy of your insurance card and a signed, completed insurance form.
- On your first visit, if the charge exceeds $100 we will do a one time only courtesy file for 80 % of the total charges. Your payment portion at the time of visit will be 20% of the total charge.
- If the charge is less than $100, you will be expected to pay the full amount and we will give you the necessary information to file your own insurance.
- If you do not have any insurance you will be expected to pay the office visit in full.
- Secondary insurances are filed if we participate with your plan. Otherwise we only file if the charges are for services at the hospital.
